Forms   
In an effort to better serve the citizens of Yuma County, many of the department forms are now available online.  This will enable citizens to complete the forms in the comfort of their own home prior to visiting the Sheriff's Office.  This will also promote speedier customer service.

In addition, we are also providing Sheriff's Office informational brochures and crime prevention brochures.

Unless otherwise specified, the above listed forms are in PDF format and you must have Adobe Acrobat Reader installed on your PC in order to view and use these forms.  Acrobat Reader is available for free download using the link provided. PDF forms are provided in two categories:

Fill-In: Using Adobe Acrobat Reader, you will have an input mask to fill in the appropriate information on screen prior to printing. When using a fillable form, the tab or mouse will advance the cursor from field to field. This form can then be printed and signed for the hardcopy submission. While this feature may provide some convenience for the user as it eliminates the need for a typewriter, proofing must be done prior to closing the file as all filled in information will be lost and only the base unfilled form will remain. The only way that the information in the individual filled in forms can be saved for later editing and/or archive reference is if the user owns a licensed version of Adobe Acrobat or Business Tools and has it installed on their computer.

Non-Fill-In: Available for information purposes or as a master for printing copies as necessary to be completed by hand or typewriter.
